You should also include the full name of the trust or entity that will receive and manage your assets if you become disabled. You should also include the names, contact numbers, and addresses of the people who are legally authorized to make decisions for that trust or entity. Finally, you should sign and date your Letter of Intent to make it legally binding. It is a good idea to keep a copy of your Letter of Intent somewhere safe to ensure that it is easily accessible in the event of your disability.
