How do I apply for a Guardianship?
In Arizona, the application for guardianship must be completed and filed with the court in the county where the disabled person resides. The process for guardianship requires that several forms be completed and signed. First, you must complete the Appointment of Guardian Application and provide proof of identity and residence. The application must include a medical report from the disabled person’s doctor detailing the disability. You must also provide an itemized list of assets and income of the disabled person, as well as a plan for the management of these funds and property. After you have completed and signed the required guardianship forms, you must make arrangements for advertisement of the guardianship in a local newspaper. This advertisement must list the filing date of the guardianship and inform the court of the time and place of the guardianship hearing. You must also provide notice to the disabled person and any other interested parties. Finally, after you have filled out all the required forms and provided notice of the guardianship, you must attend the hearing with the court. At the hearing, you must demonstrate to the court why the guardianship should be granted. During the hearing, you may be asked to provide testimony, witnesses, or other evidence demonstrating the need for the guardianship. The court will then make a decision about whether to grant the guardianship.
