What is an Advanced Medical Directive?

An Advanced Medical Directive (AMD) is a legal document that outlines a person’s wishes regarding healthcare decisions in the event that they are unable to make them for themselves. This document establishes who the patient has legally appointed to make medical decisions on their behalf, how those decisions are made, and if the patient wishes to be kept alive by “artificial means” (such as a feeding tube). It also states a person’s wish to donate organs or tissues and any instructions about funeral arrangements. In South Carolina, the AMD must meet the requirements set out in the state’s Disabilities Planning Laws, which include being witnessed by two disinterested persons or notarized. Once a person has completed an AMD and it has been notarized, the document remains in effect until the person revokes it or a court declares them to be legally competent. The document should be kept in a safe place and given to the person’s healthcare provider to keep on file. Furthermore, the healthcare provider should be informed of any changes to the document to ensure that the patient’s wishes are properly reflected. Lastly, the document should be shared with any family members or loved ones so that they are aware of the person’s wishes.
