What documents do I need to apply for disability benefits?

If you live in the state of Washington and are interested in applying for disability benefits, there are several documents you will need to have in order to do so. These documents include proof of identity, such as a state-issued driver’s license or ID card; proof of income, such as recent pay stubs; proof of residency, such as a recent utility bill; medical documents, such as doctor’s notes or test results; and a completed application for disability benefits. In addition, the State of Washington requires that you provide information regarding your financial resources, including any investments, savings, or pensions you may have. This includes any resources held by a spouse or other household members. You may also need to provide documents about your medical condition, such as doctor’s reports, x-rays, physical therapy evaluations, or hospital discharge summaries. Once you have gathered these documents and completed the application, you may be asked to attend a face-to-face interview with a representative from the department of Social and Health Services. You will have to provide even more information during this interview, such as details about your medical condition and how it impacts your ability to work or take care of yourself. This information will be used to determine your eligibility for disability benefits. After your application and documents have been reviewed, you will receive a determination of your eligibility. If you are approved, you will begin receiving benefits as soon as possible.
