What is a Nursing Home Care Plan?

A Nursing Home Care Plan is a document that outlines the care that someone in Washington receiving long-term care services will receive. These plans are developed by the nursing facility that is providing the care as well as the individual receiving the care, their family members, and health care professionals. The plan will be tailored to the individual’s unique needs, care requirements, and preferences. Nursing Home Care Plans focus on four key areas: Physical Well-Being, Social Well-Being, Cognitive Well-Being, and Self-Sufficiency. Physical Well-Being includes providing the necessary medical treatment, as well as providing for safe living conditions. Social Well-Being includes providing activities and communication to help build meaningful relationships. Cognitive Well-Being includes providing the resident with adequate mental stimulation and ways to maintain cognizance. Self-Sufficiency includes helping the resident maintain the highest level of independence possible. The Nursing Home Care Plan is an important document in Washington State’s Disability Planning Law. It outlines the services and care that each resident should receive and how they will be provided. The plan is revised regularly to ensure that it meets the changing needs of the individual recipient. This ensures that the elderly in Washington are receiving the necessary care and services they need and deserve.
