What is a health care proxy?

A health care proxy is a document that is used in Rhode Island for disability planning law. The purpose of a health care proxy is to appoint a person (or persons) of your choice to make health care decisions on your behalf in the event that you become unable to make those decisions yourself due to physical or mental disability. The person you choose to appoint as your health care proxy is referred to as your “Health Care Agent” or “Health Care Representative.” Your Health Care Agent is legally authorized to make decisions on your behalf regarding your medical care or treatment. This includes decisions related to medical treatments, end-of-life decisions, and other important health care matters. You can designate more than one person to serve as your Health Care Agent, and can give them different levels of authority to make decisions. In Rhode Island, your health care proxy must be over the age of 18 and must be deemed legally competent. It is important to name someone trustworthy and reliable who shares your values and wishes, and who you can trust to make decisions on your behalf. By creating a health care proxy in Rhode Island, you are ensuring that your wishes are respected and that someone familiar with your values and wishes will be making important medical decisions on your behalf. It is important to review your health care proxy regularly and make updates when necessary.
