What is a health care directive and how does it work?

A health care directive, also known as an advance directive, is a document that outlines the medical treatment you want to receive in the event that you become unable to make decisions for yourself. In South Carolina, a health care directive can be either an Advance Directive for Medical Care or an Advance Directive for Mental Health Treatment. The Advance Directive for Medical Care lets you express your wishes about medical treatments and end-of-life care. It includes information about whether and when you want to receive CPR, tube feeding, dialysis, and other medical treatments. It also includes the names of individuals you want to make health care decisions for you if you’re unable to make them for yourself. The Advance Directive for Mental Health Treatment lets you specify the type of mental health treatment you want to receive. It can also be used to name individuals who can make decisions about your mental health care if you are unable to do so. In South Carolina, both of these documents must be signed and dated in front of two witnesses, or a notary public. After that, you should provide copies of these documents to your physician, hospital, and family members. This will ensure that your wishes about your medical care are respected in the event that you become unable to make health care decisions for yourself.
