How do I apply for survivor benefits?
If you are a survivor of a veteran who has passed away, you may be eligible for survivor benefits in Washington. To apply for survivor benefits, you must first complete and submit an Application for Dependency and Indemnity Compensation (VA Form 21P-534EZ) to the Department of Veterans Affairs (VA). You may submit the form online, via mail, or in person at a VA regional office. When submitting your application for survivor benefits, you must also include certain documents and information. Specifically, you should provide a copy of the veteran’s death certificate, proof of marriage to the deceased veteran, proof of any dependents, and proof of any income or assets. You must also include your Social Security number, your current address, and your bank routing and account number. After your application is submitted and reviewed, you will be notified of the VA’s determination regarding your eligibility for survivor benefits. If you are deemed eligible, you will receive periodic payments over the course of a year. When the year is up, you must reapply for survivor benefits.
