What are the requirements and procedures for an aircraft accident investigation?

In Oklahoma, aircraft accident investigations are conducted following a crash of an aircraft. The National Transportation Safety Board (NTSB), the Federal Aviation Administration (FAA), and the Oklahoma State Police report to the scene of a crash to begin an investigation. The NTSB is responsible for gathering evidence and determining the probable cause or causes of a crash. It will investigate the human, mechanical, environmental, and other factors that influenced the accident. The FAA enforces compliance with safety regulations and inspects aircraft components. The Oklahoma State Police will assist by controlling the area of the accident and securing the site. In the event of an aircraft accident, the NTSB will lead the investigation and collect evidence to determine the probable cause of the crash. There are four steps for the NTSB to follow: Accident Investigation; Recording & Analysis; Formulation of Findings; and Public Report & Safety Recommendations. Accident Investigation involves reviewing documents, conducting interviews, and examining the scene of the accident. Recording & Analysis focuses on collecting data and meticulously documenting the accident. During Formulation of Findings, the NTSB will use the collected evidence to identify the probable cause for the accident. Lastly, the NTSB will issue a Final Accident Report and Summation of Findings that will include safety recommendations. The NTSB will then provide the report to the FAA and the Oklahoma State Police. The FAA and State Police will then use this information to take any necessary steps to ensure the safety of future aircraft operations.
