What is a Nursing Home Care Plan?

A Nursing Home Care Plan is a plan of care created by a doctor when a person is admitted to a nursing home and is designed to ensure that the person receives the best possible care and quality of life while in the nursing home. It is a tailored plan that is individualized to meet the person’s specific medical, physical, dietary, and emotional needs. The Care Plan is based on an assessment of the person’s current situation and medical history and includes the nursing home staff’s opinion of the person’s current needs. It is also based on input from the patient, as well as any family or friends who may be involved in the decision-making process. The Care Plan is designed to ensure that the person receiving it is receiving appropriate medical care and that their needs are being met in the most effective way. In Rhode Island, care plans must meet with the requirements of the Nursing Home Care Plan Regulations, which include everything from the care provided by the nursing home staff to the development of an individual service plan. Additionally, nursing homes must provide discharge planning, medical care, and other services as outlined in the rules. The care plan must also be updated as the person’s needs change and must be available for review by the resident and their family.
