What is a Nursing Home Care Plan?
A Nursing Home Care Plan (NHCP) is a state-approved program that outlines the care and services someone with a disability needs in order to live in a nursing home. This plan may include the assistance and services needed for daily activities such as bathing, dressing, housekeeping, nutrition, and medical care. Additionally, this plan may include information on supports to help with communication, mobility, or other activities of daily living. In Texas, the Nursing Home Care Plan is guided by the state’s Disability Planning Law, which provides protection and access to long-term care services for people with disabilities. Under the Texas Disability Planning Law, nursing homes must provide a personalized plan of care to meet the needs of individuals with disabilities. Each person’s plan must include details such as a list of services and activities that will help the person safely live in a nursing home and reach their goals. In addition, the plan should explain any special considerations related to the person’s disabilities, such as any medical equipment that may need to be provided or any therapies that may need to be incorporated. The Texas Disability Planning Law directs nursing homes to partner with individuals and their families when creating Nursing Home Care Plans, as the individual and family know best what a person needs. With the input of the individual’s family and healthcare professionals, nursing homes create a customized plan of care that considers their needs, abilities, and any other special conditions. It is then up to the nursing home to ensure that the plan is followed and the individual is receiving the services needed.
