What documents are needed to apply for a U.S. passport?

In order to obtain a U.S. passport, you will need to present certain documents. The type of documentation needed will depend on whether you are an adult applying for a new passport or a minor applying for their first passport. For both adults and minors, an application form (Form DS-11) must be completed and submitted along with proof of U.S. citizenship, proof of identity, and a passport photo. For adults, proof of citizenship can be presented with an original or certified copy of a birth certificate, Consular Report of Birth Abroad, Certificate of Naturalization or Certificate of Citizenship. Acceptable forms of proof of identity include a valid driver’s license, a valid government-issued ID, or a certified copy of a state-issued birth certificate. Minors under the age of 16 must have both parents or guardians appear with them in person when applying for a passport. In addition to the above documents, both parents or guardians must also provide proof of their relationship to the child, as well as proof of their own identity. The only acceptable forms of proof of relationship are a birth certificate, an adoption decree, or a court order establishing guardianship. Finally, regardless of age, all applicants must provide a 2x2-inch passport photo that meets the government’s requirements for passport photos.
