How do I file a complaint with the Occupational Safety and Health Administration (OSHA)?

In Washington, filing a complaint with the Occupational Safety and Health Administration (OSHA) begins with the whistleblower. To file a complaint with OSHA, whistleblowers must provide a signed and dated written complaint with their contact information. The complaint should include the date of the alleged violation, the location of the alleged violation, the employer’s name and address, a description of the violation, and any other relevant information. Whistleblowers can file their complaint in any of the following ways: • In Person: Bring the signed and dated complaint to the nearest OSHA area office. Locations can be found on OSHA’s website. • By Mail or Fax: Send the signed and dated complaint to: OSHA, 200 Constitution Ave, NW, Washington, DC 20210. Every office also has a fax number. • By Telephone: Call OSHA’s toll-free hotline (1-800-321-6742) and provide the required details over the phone. • Online: The whistleblower can go to OSHA’s website (www.osha.gov) and submit the signed and dated complaint online. After submitting the complaint, the whistleblower should receive a response from OSHA within five business days. OSHA will investigate the complaint and may ask for additional information or documents. Depending on the type of complaint, the investigation can take up to six months to complete. After the investigation is complete, OSHA will inform the whistleblower of the results.
